Cavern Run recreates Scramble, released by Konami in 1981 and generally counted the first side-scrolling shooter built as a run of distinct stages rather than one endless field.
Fuel is the real clock. You burn it constantly, and the only way to top it up is to bomb the fuel tanks on the cavern floor. Those tanks sit in exactly the awkward places, low and tight against the rock, so staying alive means repeatedly diving into the most dangerous part of the cave. Flying safely high above the danger is a slow way to run dry.
